Type B3 Fractures

Severe proximal femoral bone loss makes it even more challenging to achieve good femoral component and fracture fixation as is seen in a Vancouver B3 periprosthetic fracture (Fig. 11.4). Options for treatment of such challenging fractures include long cylindrical or fluted stems, with or without cortical strut grafting, allograft-prosthetic composite, or proximal femoral replacement. The optimal method of reconstruction depends on the patient’s physiologic demands, fracture extent and location, and degree and severity of bone loss.
